HIV/Hepatitis C Bridge Counseling

Bridge counseling is the process of linking patients to the care they need around specialty HIV or Hepatitis C care, supportive services, and primary care when possible.

If you suspect or have recently been diagnosed with Hepatitis C or HIV, Wake County Health & Human Services is here to help! Our team provides no-cost services through our social workers/bridge counselors, who will guide you through the process of finding care.

Wake County bridge counselors work with Wake County residents only. They follow each case up to four months to ensure clients are linked to care, engaged in care, and have access to medications.

Services offered include:

  • Education on options for care
  • Scheduling with medical providers
  • Enrolling in Ryan White supportive services
  • Completing a full social work assessment
  • Education on HIV and Hep C basics & treatment
  • Offering referrals to services such as mental health, substance abuse, nutrition counseling, case management and other services
  • Applications to other services and medication access programs
  • Services in English and Spanish

At Wake County Health & Human Services, we understand every patient’s needs are different and offer an individualized approach to patient care. Our Hepatitis C medical provider delivers high-quality medical care on-site and through our network of partners.
