Care Management for High Risk Pregnancies (CMHRP)

Care management services are available to residents who are pregnant or in their postpartum period who are enrolled in North Carolina Medicaid. Our care managers provide virtual and in-person one-on-one support such as meeting you at your doctor’s appointments, home visits, community visits and telehealth visits.  

Requirements

You may be eligible for CMHRP services if you have certain medical or social conditions such as

  • High blood pressure or diabetes 
  • Previous preterm delivery (prior to 37 weeks) or other pregnancy related conditions
  • Pregnant with more than one baby
  • Have unsafe/unstable living conditions such as experiencing homelessness, abuse or violence
  • Difficulty getting to your doctor's appointments

 

Your care manager can help you by: 

  • Coordinating your care across all your healthcare providers
  • Supporting you during your pregnancy and after your baby is born
  • Helping link you with needed services and referrals for things such as medical and dental care, transportation, food and/or stable housing (when available)
  • Referring you to other programs like childbirth or breastfeeding education classes, family planning and the Women, Infants and Children (WIC) nutrition program

 

These services are part of your Medicaid insurance plan so costs you nothing!
