Population Health

Population Health works to eliminate health disparities among all populations living in Wake County.

The CDC views population health as an interdisciplinary, customizable approach that allows health departments to connect practice to policy for change to happen locally. This approach uses non-traditional partnerships among different sectors of the community—public health, industry, academia, health care, local government entities, etc.—to achieve positive health outcomes.

HealthLit4Wake aims to increase confidence and trust in local health service providers while establishing a stronger link between communities and government health agencies in Wake County. This work occurs through the bidirectional engagement of all partners.

Live Well Wake

Live Well Wake logo

Live Well Wake aims to make Wake County the healthiest community in the nation for all residents – regardless of background, neighborhood or circumstance—to be able not just to live, but to flourish. Built on the premise that everyone has a right to live well, Live Well Wake is a community-led initiative bringing people together across sectors to find innovative solutions for our county’s biggest challenges. Formed out of the Community Health Needs Assessment and Population Health Task Force initiatives, Live Well Wake is a collaborative effort that relies on our community at-large to build partnerships and design strategic initiatives to meet our goals. The work of Live Well Wake focuses on these three priority areas:

  • Access to Healthcare
  • Housing and Homelessness
  • Mental Health

HealthLit4Wake Impact Report 2025

Highlights from this year's report include launching two rounds of mini-grant opportunities allowing us to work alongside community partners, transforming our former COVID-19 Mobile Unit into a more comprehensive Public Health Services Mobile Unit (Wake Wheels for Health) and implementing a range of new strategies. These projects have allowed us to address gaps in access and increase our community’s capacity to face emerging health issues with knowledge, preparedness and collaboration.

This report reflects the efforts of our staff, partners and stakeholders. It is also a testament to the power of collaboration and community-centered planning. We are proud of what has been accomplished in the year so far and are energized for 2026!

Community Health Workers

Empowered Citizen

HealthLit4Wake has been collaborating with Southeastern Healthcare of North Carolina to hire a team of community health workers since December 2021. Currently there are 8 community health workers who are engaged with assisting Wake County residents to get access to health and social service resources. They do this through attending community health events, partnering with community organizations and institutions to table at their location and connect with clients, and assist individuals referred to them through NCCARE 360, an online platform that lists available resources in North Carolina.

Community Health Workers are trained and trusted members of the community who act as a liaison between the community and health and social services. Wake County Public Health is proud to be collaborating with Southeastern Healthcare of North Carolina on a team of community health workers who are available to assist community members to get access to a variety of resources including food, healthcare, mental health services, utilities assistance, employment services, transportation, housing services, among others.

HealthLit4Wake Mini Grant Funding

The HealthLit4Wake Mini-Grant program provides funding to community organizations working to improve health literacy and expand access to reliable health information across Wake County. This initiative supports local partners in delivering education, outreach, and community-based programs that help residents better understand health topics, connect to available services, and make informed decisions about their health. Funded projects focus on increasing awareness, strengthening community outreach, and addressing priority health needs through workshops, events and practical resources.

Educational Opportunities

Our educational opportunities provide community members and partners with access to trusted health information, practical resources, and engaging learning experiences. One of our featured offerings is the Ask the Doc Town Hall series, which connects residents with medical professionals to discuss important health topics, answer questions, and share guidance in an open, community-focused setting.

In addition to scheduled events, a variety of trainings and educational sessions are available upon request for organizations, community groups, and partners. These opportunities can be tailored to meet specific needs and may include workshops, presentations and resource sharing on a range of public health topics.

HealthLit4Wake Coalition & Workgroups

HealthLit4Wake is dedicated to enhancing health literacy in our community through collaborative efforts. The coalition meets every fourth Thursday of the month, bringing together local stakeholders to discuss strategies, share resources, and align on goals for improving health literacy across Wake County. This gathering fosters a strong network of partnerships focused on creating a more informed and healthier community for all.

In addition to the coalition meetings, HealthLit4Wake has five monthly workgroups that invite local community partners to engage in meaningful discussions about relevant issues, share innovative ideas, and plan upcoming events. These workgroups serve as a vital space for collaboration and connection, welcoming any community partners who wish to get involved. Together, they work toward strengthening health literacy initiatives and addressing the unique needs of our community.

Policy, Analytics, Capacity Building and Training (P.A.C.T.) Workgroup

The P.A.C.T. workgroup meets to drive progress across four key areas:

  1. Policy and Data Integration – Aligning policy development with data-driven analytics to ensure informed decision-making and sustainable outcomes.
  2. Enhance Analytical Capacity – Strengthen the ability to collect, analyze, and apply data effectively to support evidence-based policies and initiatives.
  3. Build Organizational Capacity – Provide comprehensive resources and strategic support to enhance the operational capabilities of stakeholders and partners.
  4. Offer Specialized Training – Deliver targeted training programs designed to enhance expertise in policy, data analysis, sustainable practices and capacity building.

Community Outreach and Engagement Workgroup

This workgroup will develop strategies for engaging communities in health literacy work by discussing strategies for enhancing health messaging for a range of topics, with a particular focus on communities. This collaboration ensures our communications are culturally sensitive and relevant. Additionally, the workgroup reviews health training materials, offering constructive feedback to improve their effectiveness. Through these efforts, we aim to elevate the overall quality of health messaging and education, fostering better health outcomes for all.

Youth Engagement Workgroup

The Youth Engagement Workgroup is dedicated to developing strategies for involving youth in health equity and health literacy initiatives. This group discusses and shares updates on the Wake County Youth Health Council, ensuring that young voices are heard and integrated into our efforts.

In addition to fostering youth involvement, the workgroup collaborates on health training opportunities available to young people and discusses various events aimed at promoting health literacy. We also explore volunteer opportunities that empower youth to take an active role in their communities and work towards engaging them in public health roles. Through these collaborative efforts, we aim to inspire and equip the next generation to contribute to health equity and literacy in meaningful ways.
